Initial sessions are 75 minutes and $35 additional fee. Follow up sessions are 50 minutes.

Sliding Scale Fees- are based on Household income and require proof of income. Appropriate documentation include prior year tax return or pay stubs for adult wage earners in household.
Scholarships are available-please note that scholarships are funded by taking 5% off the fees from those clients paying the standard fee as well as grants and donations.
Insurance We choose NOT to be "preferred providers" for any insurance plans due to the restrictions and detailed reporting of your confidential information required. Most PPO and many HMO plans have provisions for "out of network" mental health benefits. Please refer to insurance and billing instructions below for more information.
We DO offer a sliding fee scale based on income as well as scholarships based on need. Each member of our group is committed to a belief that therapy is an important tool that should be accessible to all individuals, regardless of financial situation. Please refer above to our fees for more information. |

Most PPO and many HMO plans have provisions for "out of network" mental health benefits. We encourage you to contact your insurance company and ask these questions: 1. Do I have out of network mental health benefits? 2. How much is my deductible? 3. How much of the deductible has been met? 4. What is the maximum number of psychotherapy/counseling visits per year? 5. What is the maximum amount that can be paid per year? 6. Is there any preauthorization or referral required? If so who must provide the referral? 7. What address should I mail claims to? 8. Is there a special form that claims need to be sent in on? Is it the HCFA 1500? 9. What is the fee schedule for the following CPT codes in zip code 32819 for a Masters Level Provider: 90801-Initial Assessment Visit 90806-50 minute follow up visit 90808-75 minute follow up visit 10. What percentage of fee schedule do you reimburse for out of network visits?
Most "out of network" benfefits will pay 50-75% of the "usual and customary charges" and if you come to us with this information we can work with you so that your out of pocket cost is minimal. |
